Psychosis: case reportA 62-year-old woman developed psychosis while

receiving ropinirole for parkinsonism.The woman, who had an 18-year history of bipolar

affective disorder treated with lithium, was given tabletropinirole 5 mg/day for parkinsonism [route not stated].Later, valproic acid was substituted for lithium afterdetection of high serum lithium levels. While receivingropinirole and valproic acid she developed elementaryauditory hallucinations, delusion of perception anddecreased sleep [time to reaction onset not stated]. Shewas seen by a psychiatrist after symptoms persisted for2 weeks and was prescribed quetiapine; however, hersymptoms continued to worsen for a period of 2 months. InDecember 2009, 1 year after starting ropinirole, a mentalstatus examination revealed that she was speakingirrelevantly and had blunted affect and persecutorydelusions as well as elementary delusions with preservedcognitive functions.

Ropinirole was suspected and withdrawn; the woman’squetiapine dosage was increased while valproic acid wasunchanged. Within 2 weeks all her symptoms subsided.For the 2 months prior to last observation, she waseuthymic.
