Reactions 1340 - 26 Feb 2011 Ropinirole Psychosis: case report A 62-year-old woman developed psychosis while receiving ropinirole for parkinsonism. The woman, who had an 18-year history of bipolar affective disorder treated with lithium, was given tablet ropinirole 5 mg/day for parkinsonism [route not stated]. Later, valproic acid was substituted for lithium after detection of high serum lithium levels. While receiving ropinirole and valproic acid she developed elementary auditory hallucinations, delusion of perception and decreased sleep [time to reaction onset not stated]. She was seen by a psychiatrist after symptoms persisted for 2 weeks and was prescribed quetiapine; however, her symptoms continued to worsen for a period of 2 months. In December 2009, 1 year after starting ropinirole, a mental status examination revealed that she was speaking irrelevantly and had blunted affect and persecutory delusions as well as elementary delusions with preserved cognitive functions. Ropinirole was suspected and withdrawn; the woman’s quetiapine dosage was increased while valproic acid was unchanged. Within 2 weeks all her symptoms subsided. For the 2 months prior to last observation, she was euthymic.
