Horner’s syndrome: case reportA 33-year-old woman developed persistent, unilateral

Horner’s syndrome after the administration of bupivacaineand fentanyl [dosages not stated].

The woman received an epidural block with bupivacaine0.125% and fentanyl 2 mcg/mL during labour andunderwent a caesarean section for delivery. Uponadministration of the bupivacaine and fentanyl, sheexperienced immediate right-sided back pain.Approximately 12 hours after the administration andpostoperatively, right-sided blepharoptosis was noticed[exact time to reaction onset unknown]. She presented withpersistent blepharoptosis 9 weeks after her caesareansection.

The patient’s cranial nerves and neurological exam werenormal. She was diagnosed with Horner’s syndrome. Uponexamination, she had right-sided blepharoptosis of 1mm,right-sided proptosis of 2mm and anisocoria. There was nopupillary response in her right-eye after the installation ofcocaine 2%.
