ತಂದೆ / ಗಂಡನ ಹೆಸರು,

ಮೊಬೈಲ್ ಸಂಖೆಾ , ಹುಟಿ್ಟ ದ

ದಿನೋಂಕವನ್ನು ನಮೂದಿಸಿ

ಮತ್ತು ಜಾತಿ, ಜಿಲಿ್ಲ ,

ತಾಲಿ್ಲಕು, ಗ್ರಾ ಮ,

ಎಮ್ಎಲ್ಎ ಕೆಷ ತಾ , ಎೋಂಪಿ

ಕೆಷ ತಾ ವನ್ನು

ಆಯ್ಕಕ ಮಾಡತಕಕ ದ್ದು

Select District ,Taluk and Enter Ration card number.

ಜಿಲಿ್ಲ , ತಾಲಿ್ಲಕು ಆಯ್ಕಕ

ಮಾಡಿ ಮತ್ತು ರೇರ್ನ್ ಕಾರ್ಡ್

ಸಂಖೆಾ ನಮೂದಿಸತಕಕ ದ್ದು .
