After completing data entry, form will be given

Read form and check name, name’s spelling, birth place, birth day, gender, religion etc`

After checking all detail, have to sign two places one for details are correct and another one is for declaration of Muhammad (peace be upon him) is last prophet.Second signature is not for non MuslimIf applicant is under 18 years then Father/Mather will sign instead of him including third place

4th Step (Counter 04 for Data Entry)

Don’t leave any document at this counter and go to counter 05
