Reactions 1323 - 16 Oct 2010 S Bupivacaine Heart arrest: case report A 60-year-old man received an ultrasound guided injection of 0.5% bupivacaine to achieve left stellate ganglion block due to chronic facial pain; the needle was positioned below the prevertebral fascia and bupivacaine 10mL was administered (5 x 2mL bolus injections). He developed mouth and eye twitching immediately following the procedure; loss of consciousness and cyanosis ensued. He experienced a generalised tonic-clonic seizure, and airway obstruction resulted with loss of palpable pulse. He had previously received several uneventful stellate ganglion blocks. He was successfully resuscitated, and returned home the next day. It was presumed that the seizures resulted in respiratory then cardiac arrest. Author comment: "The likely cause of the complication was injection of local anaesthetic into the vertebral artery". Rastogi S, et al.
