Reactions 1012 - 31 Jul 2004 S Riluzole Neutropenia in an elderly patient: case report A 71-year-old woman developed neutropenia during treatment with riluzole for amyotrophic lateral sclerosis. Three months after starting treatment with riluzole 50mg twice daily, the woman presented with a fever. Laboratory investigations revealed a haemoglobin level of 11.5 g/dL, a platelet count of 156 × 10 3 /mm 3 and a WBC count of 1.24 × 10 3 /mm 3 , with 18% neutrophils (absolute count 0.22 × 10 3 ), 46% lymphocytes and 27% monocytes. Treatment with filgrastim and empiric antibacterials was started, and riluzole was discontinued. On hospital day 4, her WBC count increased to 7.33 × 10 3 /mm 3 , with an absolute neutrophil count of 5.22 × 10 3 . Filgrastim was stopped and, on day 6, her fever resolved. Author comment: "Although several infections can cause leukopenia, the lack of detection of such disease, the selective granulocytic depression, and the recovery of neutrophil count after riluzole discontinuation make drug effect more likely."
