Reactions 1406 - 16 Jun 2012 S Bupivacaine Intracranial subdural haematoma in an elderly patient: case report A 73-year-old woman developed an intracranial subdural haematoma following spinal anaesthesia with bupivacaine. The woman was admitted for arthroscopic knee surgery. She underwent spinal anaesthesia with a 2.3mL injection of 0.5% bupivacaine between L4–5. Her surgery and postoperative course were uneventful. However, on postoperative day 5, she developed a headache with vomiting at 6:00am. Her headache persisted and she appeared somnolent, while examination revealed a BP of 232/103mm Hg. The woman received pentazocine. At 9:00am, a CT scan revealed a subdural haematoma from her left frontal to temporal region. She was treated conservatively with BP control, rest and an antihaemorrhagic. After 7 days of rest, her headache resolved. On day 12 after onset, a CT scan showed haematoma absorption. One month later, a repeat CT scan revealed further reduction in the haematoma volume. On postoperative day 49, she was without subjective symptoms, and was discharged.
