Reactions 417 - 5 Sep 1992 Bupivacaine Seizures in children: 2 case reports Tonic clonic seizures developed in 2 girls following extended continuous infusions of bupivacaine for postoperative pain relief. Patient 1, a 3-year-old girl, experienced 2 seizures 21 hours after the initiation of an intrapleural bupivacaine infusion 0.25-0.5 mg/kg/h. The second patient, a 9-year-old girl, experienced 3 seizures beginning 56h after the initiation of an epidural bupivacaine infusion 1.25 mg/kg/h with epinephrine. The blood levels of bupivacaine were 5.6 and 5.4 µg/ml in patient 1 and 2, respectively. Bupivacaine was promptly discontinued in both patients and IV phenobarbital given to control seizures. The remainder of the hospital stay was uneventful in both patients. [See also Views and Reviews, p4]. Agarwal R, et al.
