Reactions 1314 - 14 Aug 2010 S Bupivacaine/lidocaine Dizziness, seizure activity and heart arrest treated with lipid emulsion: case report A 28-year-old man experienced dizziness, generalised seizures with tonic-clonic activity and heart arrest after receiving lidocaine/bupivacaine during surgical implantation of a cardioverter defibrillator. He was treated with a lipid emulsion. The man received a 50cc injection to the left subpectoral area, which contained a combination of 2% lidocaine and 0.5% bupivacaine, corresponding to total doses of 570mg and 107mg for lidocaine and bupivacaine, respectively. He developed dizziness after axillary venous access was achieved. Generalised seizures with intense tonic-clonic activity occurred several minutes later. Midazolam and lorazepam were initiated in addition to supportive ventilation. The man was subsequently intubated; pulseless electrical activity was observed and resuscitation was carried out. Epinephrine [adrenaline], vasopressin, atropine and saline were initiated. Bupivacaine toxicity was suspected, and two units of 20% lipid emulsion were infused. His pulse returned, and haemodynamic stability ensued. His surgery was uneventfully performed 1 week later, without bupivacaine. Author comment: "[W]e are confident the events were secondary to local anesthetic toxicity due to the timing, the clinical presentation consistent with local anesthetic toxicity . . . this patient received a toxic dose of lidocaine plus a dose of bupivacaine approaching the upper limit."
