Reactions 1051 - 14 May 2005 S Bupivacaine/ropivacaine Intracerebral haemorrhage: case report A 50-year-old woman developed an intracerebral haemorrhage following spinal-epidural anaesthesia for haemorrhoidectomy. The woman received bupivacaine 10mg intrathecally for anaesthesia, and a continuous ropivacaine infusion [dosage and duration of treatment not stated] via an epidural catheter for postoperative analgesia. She also received SC enoxaparin sodium on postoperative day 1. On postoperative day 2, she was discovered unconscious and biting her tongue. A brain CT scan showed a recent frontal haematoma with subarachnoid haemorrhage. She recovered normal function 12 hours later. [Treatment given not stated.] Author comment: It is questionable whether the combined spinal anaesthesia is the direct and unequivocal cause of the intracerebral haemorrhage. Albaladejo P, et al.
