Reactions 1364 - 13 Aug 2011 S Bupivacaine/sufentanil Abducens nerve paralysis treated with mannitol: case report A 31-year-old woman developed abducens nerve paralysis after spinal anaesthesia with bupivacaine and sufentanil; she was successfully treated with mannitol. The woman was scheduled for a caesarean section, and received bupivacaine 10mg and sufentanil 2.5µg via a 26-gauge needle into the L3-4 interspace. Surgery was performed uneventfully, and she was discharged the next day. However, she presented to a hospital 2 days later with a postural headache. The woman received sodium chloride, paracetamol [acetaminophen], aspirin, caffeine and ibuprofen. Her headache improved 24 hours later; however, she developed diplopia. Neurological, ophthalmological and radiological tests, including brain MRI and 3D retinal scans, yielded normal results. Fifteen days after surgery, she received an IV infusion of 20% mannitol 1 g/kg over 30 minutes. Her diplopia improved within 2 hours, and her vision normalised after 8 hours.
