Abducens nerve paralysis treated with mannitol:case report
A 31-year-old woman developed abducens nerveparalysis after spinal anaesthesia with bupivacaine andsufentanil; she was successfully treated with mannitol.
The woman was scheduled for a caesarean section, andreceived bupivacaine 10mg and sufentanil 2.5µg via a26-gauge needle into the L3-4 interspace. Surgery wasperformed uneventfully, and she was discharged the nextday. However, she presented to a hospital 2 days later witha postural headache.
The woman received sodium chloride, paracetamol[acetaminophen], aspirin, caffeine and ibuprofen. Herheadache improved 24 hours later; however, shedeveloped diplopia. Neurological, ophthalmological andradiological tests, including brain MRI and 3D retinal scans,yielded normal results. Fifteen days after surgery, shereceived an IV infusion of 20% mannitol 1 g/kg over30 minutes. Her diplopia improved within 2 hours, and hervision normalised after 8 hours.
