Reactions 1362 - 30 Jul 2011 O S Theophylline overdose Status epilepticus and amnesia: case report A 55-year-old man, who had a history of mild obstructive pulmonary disease, developed a series of tonic-clonic seizures without interictal reorientation for more than 1 hour. On admission, he was disoriented and required mechanical ventilation. It was revealed that he had accidentally ingested a high dose of theophylline the night before [dosage not stated]. His theophylline concentration was 58 mg/L. Following charcoal administration and forced diuresis, no further seizure activity was seen. Five days after admission, a MRI showed increased signal intensities in the hippocamal region. After 3 months’ rehabilitation, he had persistent long-term memory deficits. At last follow-up, he had improvements in working memory, but changes in long-term memory function persisted. Author comment: "We describe a patient who developed a severe amnesic syndrome after theophylline-induced [status epilepticus] with selective bilateral hippocampal damage." Kohl Z, et al.
