Reactions 1046 - 9 Apr 2005 Atenolol/imidapril Pemphigus (first report with imidapril) and atenolol- induced lupus in an elderly patient: case report A 67-year-old woman developed lupus during treatment with atenolol, and later developed pemphigus during treatment with imidapril for hypertension. The woman presented with a 1-year history of photodistributed telangiectatic erythema, ragged posterior cuticles and periungual erythema. Laboratory investigations revealed positive antinuclear, anti-dsDNA and antihistone antibodies, consistent with drug-induced lupus. Several months prior to the onset of her symptoms, she had started treatment with atenolol [dosage not stated]. Atenolol was discontinued and, after 3 months, the woman’s skin eruption had resolved and her autoantibody levels had normalised. Almost 5 years later, 2.5 years after starting treatment with imidapril [dosage not stated], she developed crusted plaques and erosions over her scalp, arms and upper back. Examination, histology and immunofluorescence were consistent with pemphigus foliaceus. Treatment with prednisolone and hydroxychloroquine was started and imidapril was discontinued; 4 months later, her skin eruption had improved significantly. Subsequent HLA typing revealed HLA-DR4. Author comment: "At this point we can only surmise that the haplotype HLA-DR4 predisposes to a unique drug metabolism that results in the development of these two rare drug-induced immune-mediated dermatoses."
