Reactions 1306 - 19 Jun 2010 S Bupivacaine Post-dural puncture headache leading to subdural haematoma: case report A 39-year-old woman developed post-dural puncture headache (PDPH), leading to cerebral subdural haematoma after receiving spinal anaesthesia with bupivacaine for a caesarean section. The woman underwent elective caesarean section with 0.5% bupivacaine 12mg. The procedure was uneventful, apart from a drop in her systolic blood pressure requiring ephedrine. Three days later, she reported a mild occipital headache that worsened in a sitting position; PDPH was suspected. With bed rest and hydration, her headache rapidly improved over the following 2 days, and she was discharged 5 days after her caesarean. The following day, she developed headache with fifth nerve palsy and tearing of her right eye. Intense orbital pain on her right side and diplopia developed on post-operative day 15, and she was hospitalised on day 30 after developing left-sided weakness. On examination, left-sided limb weakness was observed, as well as severe non-postural headache and right eye pain. Cerebral subdural haematoma involving her right fronto-parietal lobe was detected on CT scan, and was estimated to be more than 2 weeks old. The woman received conservative treatment, and her symptoms subsequently improved. Repeat CT scan revealed resolution of her haematoma 11 days later, and she was discharged the following day. A full recovery was noted 12 weeks following dural puncture. Author comment: "In conclusion, our patient developed a cranial subdural hematoma following untreated PDPH." Zeidan A, et al.
