Reactions 1405 - 9 Jun 2012 S Citalopram Optic neuritis (first report), mydriasis and galactorrhoea: case report A 25-year-old woman developed mydriasis, optic neuritis and galactorrhoea while receiving citalopram. The woman, who presented in October 2008 with moderate depression, started receiving citalopram at increasing doses (20–60mg) [route and frequency not stated] due to ineffectiveness of opipramol. Despite a slight improvement in her depressive symptoms, she was admitted in November because of undulating mood. Zolpidem and mirtazapine were administered. Marked mydriasis was noted within 2 months of citalopram treatment. She also reported paraesthesias, dysaesthesias and left sided headaches. A left-sided acute optic neuritis with paracentral scotoma was diagnosed. The woman started receiving methylprednisolone. A very small patent foramen ovale was observed on echocardiography, and aspirin was recommended. Gynaecological examinations showed left-sided lactation. She also tended towards increased blood glucose levels, which normalised after steroid discontinuation. Ankle oedema developed, but resolved following furosemide treatment. Her ocular symptoms had resolved after citalopram cessation, and bromocriptine was initiated due to a persistence of her galactorrhoea.
