Reactions 1284 - 16 Jan 2010 Citalopram Clitoral priapism in a newborn following in utero exposure?: case report A newborn girl developed clitoral priapism following in utero exposure to citalopram [exact dosage and duration of exposure not stated]. The girl was referred for an endocrinological consultation following the discovery of clitoromegaly during an examination 2 hours after birth. She was very restless, with weak sucking postpartum. Examination 24 hours postpartum showed no pathology other than clitoromegaly. The clitoris was hard, erectile and 16.4mm in length; it was observed that she cried more intensely during clitoral examination. It was discovered that her 29-year-old mother had taken an antidepressant containing citalopram after being diagnosed with major depression before pregnancy. She had taken the drug irregularly during her pregnancy, and had been taking 20–30 mg/day of citalopram for the last 2 months due to increased depressive symptoms. Repeat examination of the neonate 48 hours postpartum showed her sucking was active, and that her clitoris had became smaller and softer with a length of 6.4mm. All endocrine and biochemistry tests were normal. Her clitoromegaly disappeared within 2 days, during which all laboratory findings remained normal. It was concluded that her clitoral priapism may have resulted from her mother’s usage of citalopram. Author comment: Citalopram inhibits the reuptake of serotonin and elicits serotonergic activity; accordingly, it is considered to cause priapism in this way. Oguz Kutlu A, et al.
