Reactions 1364 - 13 Aug 2011 S Citalopram Parkinsonian syndrome in an elderly patient: case report A 67-year-old woman developed Parkinsonian syndrome while receiving citalopram for depressive syndrome. The woman, who had a history of arterial hypertension and heart arrhythmia, presented to the emergency department with acute-onset gait difficulties and "generalised slowness". She had first developed symptoms approximately 2 weeks after the introduction of citalopram 20mg once daily [route not stated]. Examination revealed a hypophonic voice, reduced facial expression, and bilateral rigidity and bradykinesia, which were more severe on her left extremities. Mild resting and postural tremor of her left hand were apparent, and she walked with short steps, forward flexion of the trunk, and a reduced arm swing. She was unable to maintain the balance on a retropulsion test, and achieved a score of 36/108 on the Unified Parkinson Disease Rating Scale (UPDRS). She was referred for a second opinion and re-examined 1 month later where she scored 4/108 on the UPDRS, consistent with a full recovery. The woman reported that her symptoms had improved within 3 days of discontinuing citalopram, and had completely resolved 1 week later. Author comment: "The Naranjo Adverse Drug Reactions Probability scale was applied, and the patient’s score was 8, indicating a high probability of adverse drug reaction." Miletic V, et al.
