Reactions 1319 - 18 Sep 2010 S Clomipramine/clozapine Speech disorders and tonic-clonic epilepsy: case report A 15-year-old boy with schizophrenia developed tardive speech dysfluency and generalised tonic-clonic seizures during treatment with clomipramine and clozapine. The boy received a combined treatment for schizophrenia, consisting of clomipramine 225 mg/day and clozapine 350 mg/day [routes not stated], following persistent obsessive-compulsive symptoms and antipsychotic treatment resistance. Three years after starting combined therapy, he developed facial tic-like movements, involuntary paroxysmal perioral movements, myoclonic jerks of his arms and dysfluent speech, with transient speech rate acceleration and stutter-like repetition of syllables. Drug-induced tardive dyskinesia was suspected. The boy received vitamin E, but his symptoms did not improve; his motor symptoms persisted for 5 months. Following a generalised tonic-clonic seizure, clozapine was reduced to 300 mg/day, but he soon experienced another seizure. Electroencephalography showed frequent generalised and multifocal paroxysmal epileptiform discharges and diffuse background slowing. His motor symptoms were attributed to epileptic activity. Valproic acid was started and, within days, his motor and speech disorders had resolved. Two years later, he remained seizure-free. Author comment: "The additive effect of combined clozapine-clomipramine therapy could have induced this epileptic syndrome in a nonepileptic patient by markedly reducing the seizure threshold."
