Reactions 779 - 27 Nov 1999 S Clomipramine/sertraline Acute eosinophilic pneumonia (first report with sertraline): case report A 40-year-old woman developed acute eosinophilic pneumonia during treatment with sertraline and clomipramine for depression and anxiety. The patient presented with nonproductive cough, fever, malaise and progressive dyspnoea on exertion and, for the last 4 days, resting dyspnoea. She had been taking sertraline 50 mg/day for 1 week, clomipramine 25mg twice daily for 4 weeks and alprazolam for 14 months. A chest x-ray showed infiltrates in both lower lobes and a WBC count of 15 480 cells/ ml with 12.2% eosinophils. The woman was admitted to hospital and sertraline was stopped. The results of respiratory function tests were as follows: FVC 1580ml (53.1%); FEV1 1500ml (58.5%); FEV1/FVC 0.94 and maximum mid expiratory flow 4820ml (137%). Two days later, she developed a morbiliform rash on her trunk, back and legs and clomipramine was stopped. Her liver enzyme levels were increased. There was no evidence of parasitic or fungal infections. Over the next 5 days, the woman became afebrile, but her eosinophilia persisted (21.9%). She was discharged from hospital. One month later, she had no dyspnoea and her WBC count was 6370 cells/ml with 2% eosinophils. She began treatment with fluoxetine 5 months later. At follow-up, 1 year later, she had experienced no relapse. Author comment: ‘We believe that this is a case of drug- induced AEP [acute eosinophilic pneumonia] because other causes of pneumonitis were ruled out.’ There was strong evidence to implicate clomipramine and/or sertraline as responsible for the adverse reaction.
