Reactions 1239 - 14 Feb 2009 Ephedrine/epinephrine Salivary gland enlargement (first report with ephedrine) in an elderly patient: case report An 89-year-old woman developed submandibular salivary gland swelling after receiving ephedrine and epinephrine [adrenaline] for intraoperative hypotension while undergoing right partial hip replacement. The woman experienced an episode of severe intraoperative hypotension after receiving intrathecal bupivacaine, and another episode when the cement was placed in the medullary canal of her femur. Both hypotensive episodes were treated with ephedrine 90mg, epinephrine 100µg and normal saline. On postoperative transfer to an ICU, it was noticed that she had developed bilateral dense and painless submandibular masses [time to reaction onset not stated]. The woman was treated with oral hydration and massage with a warm compress. Her masses gradually subsided in 5 days, and she was discharged on postoperative day 14. Author comment: "We speculate that perioperative hypovolemia and the use of vasopressors may have caused the enlargement of submandibular salivary glands in this case." Pirat A, et al.
