Reactions 1133 - 6 Jan 2007 S Imipramine/prochlorperazine Neuroleptic malignant syndrome: case report A 57-year-old man with severe cancer pain began oral morphine analgesia with adjuvant prochlorperazine and imipramine [dosages not stated]. He then began to experience agitation, anxiety, insomnia and tremor [duration of therapy not stated], followed by fever (38–39°C), hallucinations, disorientation, muscle rigidity, labile blood pressure and rhabdomyolysis with a creatine kinase level of 5810 U/L (normal < 132). Neuroleptic malignant syndrome (NMS) was suspected. Prochlorperazine and imipramine were discontinued and he was treated with dantrolene. His symptoms almost completely resolved within 7 days.
