Reactions 1442 - 9 Mar 2013 S Ropivacaine Total spinal anaesthesia, cardiac arrest and paraplegia following accidental subarachnoid administration: case report A 21-year-old pregnant woman developed total spinal anaesthesia, cardiac arrest and paraplegia after accidental subarachnoid administration during epidural injection of ropivacaine. The woman, who was undergoing caesarean delivery, received epidural anaesthesia at L2–3 interspace consisting of a 3mL test dose injection of 0.894% ropivacaine, followed by a 7mL injection 5 minutes later. She had experienced paraesthesia during catheter insertion. Ten minutes after the second injection, she developed extremity analgesia, and her BP and HR decreased. Simultaneously, she could not be aroused, and she developed cardiac arrest. The woman immediately underwent CPR, ventilation and intubation. Her sinus rhythm recovered, and her BP and HR improved, but 40 minutes later, she experienced another episode of cardiac arrest. Following CPR, her sinus rhythm was restored; however, the fetal heart beat had disappeared, and the operation was cancelled. Three days later, she regained consciousness, but had developed paraplegia below T10 vertebra. Subsequently, her pain and temperature sensation improved, but her lower extremity movement did not. Author comment: "In our opinion, the occurrence of total spinal anesthesia indicated administration of 10mL 0.894% ropivacaine mesylate into the subarachnoid space.
