Reactions 1469, p42 - 14 Sep 2013 O S Verapamil/trandolapril overdose Hypotension: case report A 19-year-old woman developed hypotension following an overdose of verapamil/trandolapril [Tarka Forte]. The woman presented at an emergency clinic approximately 2 hours after ingestion of verapamil 2.4g/trandolapril 40mg (each tablet contains verapamil 240mg/trandolapril 4mg). She received a nasogastric catheter, and underwent gastric lavage with sodium chloride. She received active carbon, and was transferred to an ICU. Following a decrease in her ionised calcium level, she received calcium chloride. Since her blood glucose was over 256 mg/dL, she received insulin treatment. A central catheter was administered, and she underwent haemoperfusion. Her BP had started to decrease approximately 4 hours after admission, and she started dopamine. Due to continuing hypotension, dopamine was increased. She also received glucagon. She later received a lipid solution, containing soya oil emulsion and medium chain triglyceride. Two hours later, her systolic BP was 110 mm Hg, and dopamine treatment was decreased, then withdrawn within six hours. At follow-up, hypotension had resolved. She was discharged without sequelae. Author comment: Intravenous lipid infusion is widely accepted for the treatment of lipophilic drug poisoning, such as calcium channel blockers. . . it should be considered in particular for the treatment of verapamil poisoning, not only in late phases but also in initial treatment.
