Reactions 1318 - 11 Sep 2010 S Amitriptyline/trimipramine Tinnitus: case report A 39-year-old man developed tinnitus while receiving trimipramine for depression. His tinnitus was later exacerbated by a switch to amitriptyline. The man presented with intense bilateral tinnitus 3 days after initiating treatment with trimipramine 50mg [frequency and route not stated] in April 2008. He had a tinnitus frequency of 7000Hz and a minimal masking level with narrow band noise of 68dB hearing level. His medical history was notable for short transient episodes of tinnitus 8 years earlier. Trimipramine was initially continued at 100mg as it was deemed unlikely to have caused his tinnitus. A subsequent audiogram revealed no change in auditory function. Six weeks later, trimipramine was discontinued and amitriptyline 50mg [frequency and route not stated] initiated. However, amitriptyline exacerbated the tinnitus and was replaced by citalopram in July 2008. The man’s depression and severe tinnitus persisted. A variety of therapeutic strategies were implemented, including drug replacement with mirtazapine and hearing aids with a sound generator. However, all approaches had only limited efficacy. In March 2009 he began augmentation therapy with lithium, and he developed strategies to manage the ongoing tinnitus. Langguth B, et al. Persistent tinnitus induced by tricyclic antidepressants.
