Reactions 1338 - 12 Feb 2011 S Bupivacaine Hypotension and ventricular tachycardia following inadvertent IV administration: case report A 13-year-old girl, who was about to undergo cleft lip and palate repair, developed immediate circulatory arrest with hypotension and ventricular tachycardia following inadvertent IV administration of bupivacaine. General anaesthesia was induced with propofol, fentanyl and isoflurane and then maintained with isoflurane. Five minutes after the endotracheal tube was secured, a nurse inadvertently administered 10mL of bupivacaine 0.25% intravenously (0.5 mg/kg) instead of an antibiotic. The girl developed a sinus tachycardia at a rate of 134 beats/min and her systolic BP decreased from 90 to 60mm Hg within 90 sec. Soya oil emulsion was administered and the girl received normal saline and IV boluses of epinephrine. Her BP improved only transiently. She developed a slow heart rhythm with a broad QRS complex and ST segment depression that was immediately followed by a ventricular tachycardia. External chest compressions were commenced. The ventricular tachycardia self-converted to sinus tachycardia. Her BP was 60/35mm Hg and dopamine and epinephrine infusions were administered. Following stabilisation of her BP, she was transferred to an ICU. Pulmonary oedema developed 1 hour after the event and resolved over the following 2 days. The dopamine infusion was tapered off and she was extubated 2 days after the event. Four days after the event, she was discharged. Fuzaylov G, et al.
