Reactions 1173 - 13 Oct 2007 Citalopram Mania: case report A 43-year-old man developed mania following administration of citalopram for interferon-induced depression. The man, who was receiving peginterferon-α-2b for chronic hepatitis C, developed significant depressive symptoms. At week 6 of interferon therapy, he started receiving citalopram 10 mg/day, which was increased 1 week later to 20 mg/day. At week 14 of interferon therapy, after 8 weeks of citalopram therapy, he developed an elevated mood and citalopram was discontinued. At week 16 of interferon treatment, he developed a manic episode with flight of ideas, pressured speech, a decreased need for sleep, increased goal directed behaviours and inflated self esteem. The man received haloperidol, but his manic symptoms persisted for the remaining course of interferon treatment. At week 24 of interferon treatment, interferon was discontinued. Haloperidol was replaced by oxcarbazepine and he received supportive psychotherapy. Within 4 weeks, his manic symptoms abated. Author comment: "Here, we present a patient who developed a severe manic episode after the administration of antidepressant therapy for IFN [interferon]-induced depression while the dosage of IFN-α remained fixed." Citalopram
