Reactions 1014 - 14 Aug 2004 S Clomipramine/clozapine Dysphagia (first report with clomipramine): case report A 64-year-old man developed dysphagia and consequent respiration pneumonia during long-term treatment with clozapine for chronic schizophrenia and clomipramine for depression. The man, who had received clozapine 200–375 mg/day for approximately 10 years, along with clomipramine 75–200 mg/day, presented after an episode of choking, severe dyspnoea and dysphagia; 4 years earlier he had experienced difficulty swallowing, recurrent episodes of regurgitation, and hiccups. A chest x-ray revealed severe bilateral upper lobe consolidation with air bronchograms, consistent with aspiration pneumonia. He was transferred to the ICU, where a CT scan showed marked dilatation of his entire oesophagus, with liquid and solid material filling the lumen, and he received respiratory support for 6 days. Subsequent oesophageal videofluoroscopy showed minimal pharyngeal retention of barium and impaired oesophageal clearance. The man was treated with domperidone and received a percutaneous endoscopic gastrostomy and nutritional support. Manometry revealed severe oesophageal hypomotility and an insufficient lower oesophageal sphincter. Clomipramine was tapered and discontinued, and his clozapine dose was decreased to 300 mg/day. Six weeks later, manometry showed improved oesophageal motility and recovery of peristalsis and, at follow-up, his dysphagia had resolved.
