Reactions 1510, p29 - 19 Jul 2014 S Mepivacaine Type I hypersensitivity: case report A 54-year-old woman who received mepivacaine developed type I hypersensitivity. The woman presented after developing facial swelling, throat discomfort, dizziness and severe cutaneous itching immediately after receiving local anaesthesia with mepivacaine [route, dosage and outcome not stated]. She had developed facial angioedema after local anaesthesia 37 years earlier. Allergies to lidocaine and procaine had been confirmed in 2009, 3 years before the current presentation; however, allergy to mepivacaine, articaine and bupivacaine had been ruled out. Skin prick testing was positive for procaine, lidocaine and mepivacaine, and negative for bupivacaine and articaine. Basophil activation testing was positive for mepivacaine, and negative for bupivacaine and articaine. Author comment: "Although type I hypersensitivity reactions to amide-type local anaesthesics are rare, little is known about crossreactivity among these drugs. We present a case report with confirmed allergy, by in vivo and in vitro tests, to Mepivacaine and cross reactivity in vivo to Procaine and Lidocaine." Lizarza S, et al.
