Reactions 1426 - 3 Nov 2012 Pramipexole/ropinirole First report of spontaneous unwelcome orgasms: case report A 52-year-old woman developed spontaneous unwelcome orgasms while receiving pramipexole and ropinirole for multifocal dystonia. The woman initially received levodopa, but treatment had to be stopped because of severe diarrhoea, orthostatic hypotension, and weight loss. She subsequently started pramipexole 0.088mg twice daily [route not stated], and on the third day, she experienced four spontaneous orgasms. Each occurred about 1 hour after taking pramipexole, three during the day while doing housework, and one at night, causing her to wake up. The orgasms happened without premonition, increased sexual desire, or perceivable stimulation. She considered them as undesired and greatly embarrassing. Pramipexole was withdrawn, and the woman did not experience any further undesired orgasms. Ropinirole was started 10 weeks later. At a ropinirole dosage of 0.5 mg/day [route and time to reaction onset not stated], the spontaneous orgasms recurred, ceasing again at a dosage of 0.25 mg/day.
