Reactions 892 - 9 Mar 2002 S Ropivacaine Grand mal seizures following inadvertent intravascular injection in an elderly patient: case report A 65-year-old man experienced grand mal seizures following the inadvertent intravascular injection of ropivacaine which was being administered for axillary brachial plexus blockade. The man received 20ml of ropivacaine 0.5% (1.1 mg/kg) injected into the region of his left axilla in 5ml increments over a 1-minute period. His brachial artery was then palpated in order to insert the needle for a second injection. He suddenly lost consciousness and experienced a grand mal seizure. General anaesthesia was induced and the man was intubated. His scheduled surgery was undertaken 30 minutes later while he was under the general anaesthesia. Three hours after receiving ropivacaine, the tracheal tube was removed. A venous blood sample taken 15 minutes after the injection of ropivacaine revealed a total plasma ropivacaine concentration of 3.3 mg/L. The initial plasma concentration of ropivacaine immediately after the injection period was estimated to be 5.75 mg/L. uller M, et al.
