Reactions 1408 - 30 Jun 2012 S Trandolapril Angioedema in an elderly patient: case report A 78-year-old woman developed angioedema while receiving trandolapril. She later died. The woman presented to a general practitioner with a swollen tongue and tingling sensation in her oral mucosa and lips. A resident of a psychiatric hostel, she had a medical history that included hypertension, bipolar affective disorder and chronic renal impairment. She had developed an episode of tongue swelling 6 months earlier that had spontaneously resolved. Her medications included trandolapril 4 mg/day for the past 4 years [route not stated]. After an unremarkable examination, she was reassured and returned to the hostel. Six hours later, she presented with difficulty swallowing and a markedly swollen tongue that was protruding from her mouth. Chest auscultation detected turbulent sounds from the pharynx. The woman received hydrocortisone and promethazine, and was transferred to an emergency department. Upon arrival, her posterior pharynx was completely obscured by her large protruding tongue. Bilateral submandibular oedema was also observed. She received epinephrine [adrenaline], hydrocortisone and ranitidine, and underwent urgent fibre-optic nasotracheal intubation. She was then managed in an ICU under sedation. Cervical spine x-ray revealed complete obliteration of her oropharynx by the tongue. Her tongue oedema peaked on hospital day 2, and had almost resolved by day 6. By this time, however, she had developed ventilator-associated pneumonia, and required haemodialysis for acute renal dysfunction. She experienced acute coronary syndrome on day 8, followed by a pulmonary haemorrhage. After consulting her family, she received palliative care, and died on day 9 after extubation [cause of death not stated].
