Details required for Change Report (Section - 22)

(1) Affidavit of reporting trustee supporting the change report.

(2) True copy of Death certificate of deceased trustees / Resignation of retiring trustee.

(3) True copy of latest P.T.R.

(4) If trusteeship is hereditary (Family tree) “Pedhinamu”.

(5) Affidavit showing consent of incoming trustees.

(6) Proof for identification & residence of incoming trustees.

(7) If appointment is made by will – True copy of will, probate etc.

(8) In case of “Mahant” to “Chela”, proofs like photograph, “Chadar Vidhi”, “Bhandara” &

affidavit in support or any other documents.

(9) Resignation & affidavit of outgoing trustees.

(10) True copy of resolution & agenda.

(11) Proof regarding service of agenda to all members / trustees.

(12) For Immovable Property proof showing nature & ownership of the property like, “Gaam

Namuna Patrak -6”, 7/12, 8(A) or city survey property card, allotment letter, possession

letter, sell deed, gift deed.

(13) Certificate of approved valuer pertaining to valuation of property.

(14) In case of deletion of property order of Charity Commissioner or Joint Charity

Commissioner U/s 36 of Gujarat Public Trust Act.

(15) True copy of any order passed by revenue authority.

(16) In case of purchasing immovable property, copy of resolution passed by board of

trustees/members etc.

(17) Proof of showing sufficient fund with the trust to purchase immovable property.

(18) In case of construction on immovable property, proof of showing that it is for the

purpose of the trust. True copy of permission of local authority, copy of approved plan

& estimate.
