Reactions 1453 - 25 May 2013 O S Amitriptyline/citalopram overdose Various toxicities managed with soya oil emulsion: case report A 34-year-old woman developed various toxicities following an overdose with amitriptyline and citalopram. She was managed with soya oil emulsion [Intralipid]. The woman overdosed on amitriptyline 5600mg and citalopram 2400mg [route not stated]. She was found unresponsive [time to reaction onset not stated], and she was transferred to an emergency department. En route, she developed a tonic-clonic seizure that was treated with diazepam. She also experienced a cardiac arrest with ventricular tachycardia, which responded to a precordial thump. She received fluids and epinephrine [adrenaline]. Metabolic acidosis was seen with her blood gases, and her ECG revealed bradycardia with a prolonged corrected QT- interval and a widened QRS-complex. She received sodium bicarbonate, and she received continued supportive therapy in the ICU. She experienced further episodes of tonic-clonic seizures and recurrent ventricular ectopics that led to prolonged runs of ventricular tachycardia. The woman did not respond to magnesium and sodium bicarbonate. Therefore, she was given soya oil emulsion. Her condition improved markedly. No further seizures or arrhythmias were seen. She made a complete recovery. Author comment: "We wish to highlight the potential benefits of using [soya oil emulsion], even in late stages of severe [tricyclic antidepressant] overdose".
