Reactions 1207 - 21 Jun 2008 Bupivacaine Spinal myoclonus following spinal anaesthesia: case report A patient [age and gender not stated] developed bilateral, symmetrical, rhythmic myoclonic lower leg movements one hour after receiving 0.5% hypobaric bupivacaine 2.5mL for L4–L5 spinal anaesthesia during a Baker’s cyst excision. The severity of the myoclonic movements gradually increased, as did the frequency to about 20–30 per minute. The procedure was interrupted, and the patient was treated with diazepam initially on its own and again in combination with thiopental sodium. However, the myoclonus persisted for approximately 50 minutes, and only resolved after recovery from the spinal anaesthesia. The procedure was subsequently completed under general anaesthesia.
