Reactions 1324 - 23 Oct 2010 Bupivacaine/fentanyl Paralysis of intercostal muscles in a child: case report An 11-year-old girl developed unilateral paralysis of the intercostal muscles after receiving bupivacaine and fentanyl. The girl received postoperative analgesia through an epidural catheter consisting of 0.25% bupivacaine 10mL as an initial bolus, followed by a continuous epidural infusion of 0.1% bupivacaine and 2 µm/mL of fentanyl. The catheter was located at the T8–T9 level and the infusion rate varied between 3 and 12 mL/hour. She received the epidural subsequent to an anterior instrumented spinal fusion correcting a spinal deformity associated with adolescent idiopathic scoliosis. Later that day, she complained of light numbness in her right hand postoperatively [time to onset not clearly stated]. It was believed that the local anaesthetic in the epidural solution was a potential cause. However, the epidural analgesia was maintained due to the seriousness of the surgery. On day 3, a paradoxical chest movement was observed with diminished chest expansion on the right side. Resonant percussion was noted throughout the right lung field with reduced air entry on auscultation. It was concluded that a unilateral blockade of the intercostal nerves was caused by unilateral migration of the epidural catheter. The girl’s pain was controlled with alternative analgesics and the epidural infusion was withdrawn. After several hours, her chest movement improved significantly.
