Reactions 1270 - 19 Sep 2009 S Bupivacaine/heparin Epidural haematoma in an elderly patient?: case report A 78-year-old woman developed an epidural haematoma during endovascular aortic aneurysm repair (EVAR) including epidural anaesthesia with bupivacaine and injection of heparin [time to reaction onset not stated]. The woman had a history of hypertension and an infrarenally located abdominal aortic aneurysm extending from L4 to L5. She underwent EVAR. A needle was inserted at the L3-L4 space and the epidural space was identified. An epidural bolus of 20mL of 0.25% bupivacaine was administered; 10mL of 0.25% bupivacaine was repeated 2.5 hours later. IV heparin 5000U was administered 1.5 hours after epidural placement [therapeutic indication not stated]. Four hours after this heparin dose, the epidural catheter was removed. Her recovery from epidural anaesthesia was prolonged; her legs were still numb and she was unable to move them, 8 hours after the last epidural injection. MRI revealed an epidural blood mass, which extended for T12 to L1 and compressed part of the medulla spinalis. The woman underwent a decompressive laminectomy. Subsequent MRI showed no haematoma and no pressure on the spinal cord; however, her neurological condition showed no change. At last follow-up, 2 months later, she remained paraplegic. Author comment: "[T]he use of anticoagulants may increase the risk of neuraxial block-related epidural hematoma. . .
