Reactions 459 - 10 Jul 1993 Imipramine ECG abnormalities: incidence study The electrocardiographic (ECG) effects of imipramine were examined in a group of children with behavioural disorders including oppositional behaviour, conduct disorders, and depression. 23 children aged 5-17 years were treated with imipramine 1.5-5 mg/kg/day for 2 weeks Imipramine therapy was associated with prolongation of the PR interval, increased heart rate, and widening of the resting QRS complex. Only random changes were seen in the corrected QT interval and imipramine was not associated with clinically significant postural hypotension. Author comment: A resting corrected QT interval > 450 msec or a bundle-branch heart block should be taken as contraindication to tricyclic antidepressant therapy. Patients with a family history of sudden death, a baseline PR interval above the 90th percentile for age, or conduction abnormalities, should be carefully monitored. Fletcher SE, et al.
