★Quinapril

First report of depression in an elderly patient: casereport

An 80-year-old man with peripheral vascular disease andmild congestive heart failure developed major depression afterstarting quinapril therapy.

Soon after starting quinapril 10 mg/day, the man began toexperience a reduced appetite, insomnia, anhedonia, reducedenergy and suicidal ideation. He presented approximately 1month after the start of quinapril therapy, as his symptoms ofdepression had worsened over the past 2 weeks.

Quinapril was replaced with diltiazem and the man noted animprovement in his mood within 48 hours. Within 5 days afterstopping quinapril, his mood returned to baseline. He did notexperience any further depression after discharge.

Author comment: ACE inhibitors such as enalapril andcaptopril have previously been associated with psychiatricdisorders including depression.
