Reactions 1239 - 14 Feb 2009 Ropivacaine Narcolepsy-cataplexy episode during regional anaesthesia?: case report A 57-year-old man with a long history of a narcolepsy- cataplexy condition developed an episode of narcolepsy/ cataplexy during regional anaesthesia with ropivacaine. Bispectral index monitoring (BIS) was used to monitor for a potential narcolepsy-cataplexy episode in order to determine whether a distinctive BIS profile might be able to provide an early warning of an impending episode. BIS monitoring was initiated soon after the man was admitted to the operating theatre. He received 0.5% ropivacaine 30mL for femoral nerve block prior to lower limb surgery. No sedation was given during the surgery. Twenty minutes after BIS recording was started, he developed a narcolepsy-cataplexy episode [time to onset from drug administration not stated]; he went through three stages: a stage of prodromal intermittent low vigilance (with drifting to high muscular activity), a stage of continuous low vigilance with no muscular activity (BIS around 75), and a stage of full blown narcolepsy-cataplexy involving complete nonresponsive vigilance and complete cataplectic muscle power loss (BIS around 45 for about half an hour). There were no interventions during his episode of narcolepsy. His BIS increased with emergence from the episode at the end of surgery. He had a full recovery, and there did not appear to be any change in the duration or frequency of his narcolepsy-cataplexy episodes postoperatively. Author comment: "The purpose of presenting this report is to emphasize the fact that narcoleptic patients can still run the risk of loss of consciousness with atonia under regional anaesthesia, and such an undesirable complication cannot be under-estimated."
