Reactions 1321 - 2 Oct 2010 S Bupivacaine Meningitis following intrathecal administration: case report A 34-year-old woman developed chemical meningitis following the administration of intrathecal bupivacaine. The woman underwent successful caesarian delivery at an obstetrics clinic under anaesthesia with intrathecal 0.5% bupivacaine (10 mg) and IM pentazocine injection. Two hours after delivery, she became chilly and experienced abdominal pain and headache. The woman received pentazocine, which improved her abdominal pain. She became drowsy two and a half hours later, with a body temperature of 39.1°C and a severe headache. She was admitted to hospital. Her Glasgow coma scale score was E4V2M4 upon admission. She displayed a stiff neck, positive Kernig’s sign and mildly increased deep tendon reflexes in her lower extremities. She had elevated lactate dehydrogenase and alkaline phosphatase levels, and an elevated WBC count. An increase in cell count and total protein level were detected by cerebrospinal fluid (CSF) examination. Tests for an infectious cause were negative and brain MRI results were normal. Findings were consistent with drug-induced chemical meningitis, probably due to bupivacaine. She received flomoxef a half day post-caesarian delivery as a wound infection preventative. Her fever and headache improved, and almost completely disappeared by day 3, with improvement of the CSF abnormality. She was discharged on day 10 with almost normal CSF findings.
