Reactions 1396 - 7 Apr 2012 S Coptis Interstitial pneumonitis and acute liver injury: case report A 54-year-old man developed interstitial pneumonitis and acute liver injury during treatment with a herbal medicine, of which the main component was Coptis japonica Makino [golden thread]. The man commenced a herbal medicine containing Coptis japonica Makino in an effort to improve his blood circulation and heart function [route and dosage not stated]. Two weeks later, he developed dyspnoea, cough and chills, and stopped taking the herbal medicine. His symptoms subsequently improved and he resumed treatment, but his symptoms returned 3 days later, and he presented to the emergency department. Liver function tests were mildly abnormal, and chest imaging, bronchoalveolar lavage and lung biopsies were performed. He was diagnosed with interstitial pneumonitis and acute liver injury due to the herbal medicine. Coptis japonica Makino was withdrawn, and the man’s liver function had normalised by hospital day 6. On hospital day 7, prednisolone was started. Three days later, his symptoms improved and he was free of dyspnoea at discharge. After 6 months’ follow-up, he had experienced no further complaints.
