Reactions 1355 - 11 Jun 2011 S Ropivacaine Local anaesthetic systemic toxicity in an elderly patient treated with SMOFlipid: case report A 74-year-old man was scheduled for carotid endarterectomy. He received a modified ultrasound-guided interscalene block with perivascular ropivacaine 8mL (3 mg/mL), and developed local anaesthetic systemic toxicity. Immediately after administration, his blood pressure and heart rate increased, and he had a grand mal convulsion. He was intubated and ventilated, and received propofol, fentanyl, suxamethonium chloride and 500mL of 20% SMOFlipid (soya oil/medium chain triglycerides/olive oil/fish oil).
