Local anaesthetic systemic toxicity in an elderlypatient treated with SMOFlipid: case report
A 74-year-old man was scheduled for carotidendarterectomy. He received a modified ultrasound-guidedinterscalene block with perivascular ropivacaine 8mL(3 mg/mL), and developed local anaesthetic systemictoxicity. Immediately after administration, his bloodpressure and heart rate increased, and he had a grand malconvulsion. He was intubated and ventilated, and receivedpropofol, fentanyl, suxamethonium chloride and 500mL of20% SMOFlipid (soya oil/medium chain triglycerides/oliveoil/fish oil).
