Reactions 1402 - 19 May 2012 S Ropivacaine Grand mal convulsions: case report A 24-year-old man, who was undergoing surgical management for a deep wound, received 30mL of ropivacaine 0.75%, injected over 1 minute in 3–4mL increments, into his psoas muscle to anaesthetise the lumbal plexus. Thirty-nine seconds later, he developed a grand mal convulsion and became apnoeic with extreme salivation and lost consciousness. He underwent lung ventilation and received midazolam and pentothal. His seizures stopped after 20 seconds and spontaneous respiration returned. His operation was continued and he recovered with no further complications. The next day, a neurological examination did not reveal any abnormalities. Author comment: "We hypothesized that an intravascular injection of local anesthetic in this high dose produced this unwanted phenomenon." Noulas N, et al.
