Reactions 1409 - 7 Jul 2012 O S Verapamil/trandolapril overdose Bradycardia, hypotension and atrioventricular block: case report A 17-year-old girl developed bradycardia, hypotension and atrioventricular (AV) block following poisoning with verapamil/trandolapril [Tarka forte; time to reaction onset not clearly stated]. The girl, who had a history of psychological problems, was referred with drug poisoning, syncope and clouding of consciousness after ingesting 13 tablets of verapamil 240mg/trandolapril 4mg in a suicide attempt. The tablets had been used by her father to treat hypertension. She had received gastric lavage with activated charcoal at an outside medical centre, prior to being transferred to an emergency service approximately 7 hours after ingestion. On examination, she was lethargic, bradycardic and hypotensive. A second gastric lavage was performed. An ECG revealed complete AV block and laboratory analysis showed a creatinine level of 1.92 mg/dL. The girl was managed with sodium chloride, calcium gluconate and dopamine. A temporary pacemaker was inserted due to persistent AV block and she was hospitalised. She regained sinus rhythm and the pacemaker was removed after approximately 10 hours. She received further sodium chloride solution and glucose, and was discharged on day 3 after making a full recovery. Author comment: Sinus bradycardia, severe hypotension and AV blocks can be seen due to excessive use of combined ACE inhibitors and calcium channel blockers. Karadas S, et al.
