Abstrak

Gangguan depresi unipolar pada masa remaja umum terjadi di seluruh dunia tetapi sering

tidak disadari. Terjadi terutama pada anak perempuan, meningkat tajam setelah pubertas, dan

pada akhir masa remaja, tingkat prevalensi 11 tahun melebihi 4%. Depresi pada remaja

banyak terjadi di negara-negara berpenghasilan rendah dan menengah. Depresi dikaitkan

dengan morbiditas baik sekarang atau masa depannya, dan mempertinggi risiko bunuh diri.

Faktor risiko terkuat terjadinya depresi pada remaja adalah riwayat keluarga depresi dan

stres psikososial. Adapun faktor resikonya adalah riwayat keluarga, faktor perkembangan,

hormon seks, dan psikososial. Faktor

faktor tersebut saling berinteraksi dan meningkatkanrisiko depresi, melalui pengaruh hormonal maupun terganggunya jalur saraf. Terdapat banyak

kesamaan antara depresi pada remaja dan depresi di masa dewasa. Pada remaja penggunaan

antidepresan merupakan opini yang objektif untuk manajemen klinis. Untuk pengobatan yang

efektif pemilihan tergantung pada tingkat keparahan depresi dan sumber daya yang tersedia.

Strategi pencegahan ditargetkan pada kelompok berisiko tinggi.

Pendahuluan

Gangguan depresi unipolar adalah masalah kesehatan mental yang umum pada remaja di

seluruh dunia,1dengan perkiraan prevalensi 1 tahun 4-5% di pertengahan hingga akhir masa

remaja.2 ,3

Depresi pada remaja merupakan faktor risiko utama untuk bunuh diri, yang keduasampai ketiga penyebab utama kematian di kelompok usia ini, 4dengan lebih dari setengah

dari korban bunuh diri remaja dilaporkan memiliki gangguan depresi pada saat kematian. 5

Depresi juga menyebabkan gangguan serius sosial dan pendidikan, 6 , 7 , dan tingkat

peningkatan merokok , penyalahgunaan zat, dan obesitas. 8 , 9 Dengan demikian, untuk

mengenali dan mengobati gangguan ini adalah penting.

Depresi didefinisikan sebagai sekelompok gejala spesifik dengan gangguan yang terkait.

Gambaran klinis dan diagnosis dari gangguan secara umum mirip di remaja, dan orang

dewasa sen 10 ,11Dua sistem klasifikasi utama (klasifikasi internasional penyakit-10 [ICD-10]

dan manual diagnostik dan statistik Amerika gangguan mental -IV [DSM-IV])

mendefinisikan depresi sama, meskipun DSM-IV membuat satu pengecualian untuk anak-anak dan remaja, dimana iritasi daripada perasaan depresi diperbolehkan sebagai gejala

diagnostik inti. 12 Meskipun demikian, depresi pada remaja lebih sering terlewat dari itu

adalah pada orang dewasa,13mungkin karena keunggulan lekas marah, mood reaktivitas, dan

fluktuasi gejala pada remaja. Depresi juga dapat terjawab jika masalah presentasi utama

adalah gejala fisik yang tidak dapat dijelaskan, gangguan makan, kecemasan, penolakan

untuk bersekolah, penurunan prestasi akademik, penyalahgunaan sikap sub, atau masalah

perilaku.

Dalam beberapa hal depresi pada remaja dapat dilihat sebagai subform awal-awal dari

gangguan dewasa setara karena link yang kuat dengan kekambuhan di kemudian hari. 14

perbedaan penting ada di antara dua gangguan, terutama dalam respon pengobatan, dengan

pendapat sangat dibagi tentang praktik pengobatan terbaik. 10 Depresi pada anak-anak

prapubertas kurang umum daripada depresi pada remaja atau dewasa, dan tampaknya berbeda

dari gangguan ini sehubungan dengan beberapa penyebab, epidemiologi, dan prognosis.15 ,16

Kami fokus pada depresi unipolar pada remaja. Ketika bukti-bukti tersedia, kita fokus padagangguan depresi daripada gejalanya. Namun, dalam beberapa kasus, satu-satunya data yang

tersedia didasarkan pada studi di mana gejala depresi dilaporkan. Kasus tersebut dicatat,

meskipun ada kesamaan umumnya kuat dalam temuan penelitian untuk gejala depresi dan

depresi.

Epidemiologi

Prevalensi depresi pada anak-anak rendah (

meskipun depresi umumnya lebih sering terjadi pada anak perempuan, untuk mengenali anak

laki-laki adalah penting. Selain itu, dalam beberapa sub kelompok (misalnya, pasien dengan

gangguan perkembangan saraf dan medis) perbedaan jenis kelamin ini mungkin sangat

lemah, tidak ada, atau bahkan terbalik.37

Perubahan taksonomi dan penilaian metode pra kesimpulan yang kuat clude tentang apakahatau tidak prevalensi sejati gangguan depresi pada remaja telah berubah dari waktu ke waktu.38 Tentu saja, penggunaan layanan depresi dan pengakuan dari gangguan telah meningkat,

seperti memiliki pengobatan di banyak negara. 39 tingkat bunuh diri Pemuda berfluktuasi

selama periode yang sama. 40 Satu meta-analisis menemukan bukti peningkatan tingkat

depresi pada remaja antara tahun 1965 dan 1995.3Namun, perbandingan identik gejala layar

depresi dan kecemasan di beberapa negara.38

Hasil klinis depresi pada remaja

Studi longitudinal masyarakat dan sampel populasi berbasis klinik menunjukkan bahwa 60-

90% dari episode depresi pada remaja mengirimkan dalam waktu satu tahun. 41 , 42Namun,

dalam tindak lanjut studi 50-70% dari pasien yang mengirimkan mengembangkan episode

depresi berikutnya dalam 5 tahun. 41 , 43pasien Klinik-dirujuk umumnya tarif kurang baik

dibandingkan mereka yang dirawat di masyarakat. 6 Selain itu, pada orang dewasa sangat

sedikit individu menunjukkan gejala lengkap dan fungsional pemulihan antara episode

depresi, dengan sebagian besar laporan gejala sisa atau penurunan nilai.44 , 45

Akhirnya, depresi pada remaja juga memprediksi berbagai gangguan kesehatan mental dalam

hidup-terutama, gangguan kecemasan, gangguan-substansi terkait, dan gangguan bipolar,46 -

serta perilaku bunuh diri, pengangguran,

dan masalah kesehatan fisik.

Jadi , sebuahepisode depresi selama masa remaja sering bentara gangguan kronis atau kambuh, dan

prakiraan berbagai kesulitan psikososial dan kesehatan yang buruk.

Penyebab dan pathogenenesis

Karena penyebab heterogen dan beragam secara klinis dari penyakit, untuk memahami

patogenesis depresi pada remaja menantang. Seperti banyak gangguan kesehatan umum

lainnya, beberapa faktor risiko berinteraksi untuk meningkatkan risiko depresi dengan cara

probabilistik. Untuk menilai kontribusi setiap faktor risiko tunggal dalam isolasi dan untuk

mengidentifikasi periode perkembangan yang penting ketika paparan sangat berisiko sulit

karena banyak individu, keluarga, dan risiko sosial sangat berkorelasi dan berhubungandengan melanjutkan dan kemalangan kemudian.

Distal risiko seperti faktor warisan dan kesulitan pada awal kehidupan mungkin langsung

maupun tidak langsung mempengaruhi depresi. Risiko tersebut bisa menengahi efek mereka

melalui temperamen dan kepribadian atribut (emosionalitas negatif, penurunan emosionalitas

positif dan kontrol attentional, inhibisi perilaku, dan neurotisisme) dan kognisi. 50Namun,

bukti yang konsisten jarang apakah karakteristik tersebut faktor risiko, mediator, atau

konsekuensi dari depresi.

Faktor distal, bersama dengan perubahan hormonal dan pematangan juga dapat mengubah

sensitivitas individu terhadap stresor. Faktor risiko ini pada gilirannya mempengaruhimekanisme biologis risiko proksimal seperti perubahan pada sistem neuroendokrin dan

kemudian, modifikasi cara berpikir dan perasaan, dan pengurangan gejala depresi inti dengan

perubahan jalur biologis dengan obat-obatan.

Familial dan genetik risiko

Off musim semi dari orang tua yang mengalami depresi wajah tiga sampai empat kali

peningkatan tingkat depresi dibandingkan dengan off musim semi orang tua yang sehat. 51

Kedua gen dan faktor-faktor non-diwariskan berkontribusi terhadap risiko ini.51 , 52Meskipun

eksposur maternal depresi pada prenatal dan postnatal periode telah dianggap sebagai sangat

penting, 53 seperti pada hewan, bukti bahwa risiko untuk off musim semi dibatasi untuk

periode ini penting adalah konsisten. Penelitian longitudinal, 54 , 55 studi anak-anak yang

mental ayah kini mulai diakui, 59 efek jangka panjang pada depresi selama masa remaja

belum diselidiki.

Faktor keturunan juga berperan. Kebanyakan penelitian menunjukkan bahwa kembar depresi

menjadi semakin diwariskan dari masa kanak-kanak (nol sampai heritabilitas rendah) ke

masa remaja akhir (heritabilitas sederhana, sekitar 30-50%). 60 Tingkat heritabilitas pada

akhir masa remaja adalah sama dengan yang terlihat dalam kehidupan dewasa. 60 , 61

Beberapa penelitian telah difokuskan pada identifikasi gen-gen tertentu yang meningkatkan

kerentanan untuk depresi unipolar. Sejauh ini, tidak ada direplikasi, temuan yang signifikan

telah muncul.66 ,67Temuan negatif untuk unipolar depresi kontras dengan orang-orang untuk

gangguan kejiwaan lain seperti skizofrenia dan gangguan bipolar, mungkin karena gen

beroperasi dalam cara yang lebih kompleks untuk gangguan depresi daripada saat kita

Faktor risiko psikososial

Satu sirkuit menghubungkan amigdala ke hippocampus dan hamparan ventral dari korteksprefrontal (PFC) dan terkait dengan hipotalamus-hipofisis-adrenal (HPA) aktivitas axis.

Aktivitas di sirkuit ini tampaknya konsisten dibesarkan pada pasien dengan depresi berat, dan

keduanya didirikan dan pengobatan baru untuk depresi bisa bekerja melalui pengurangan

aktivitasnya.93Beberapa perubahan saraf juga terlihat sehat, individu yang berisiko, seperti

Faktor genetik, stres psikososial, hormon seks, dan pengembangan juga telah dikaitkan

dengan perubahan aktivitas dalam rangkaian ini, 84 , 94dengan bukti bahwa sirkuit ini jatuhtempo setelah masa remaja. Konsentrasi tinggi dari seks reseptor steroid telah diidentifikasi
