HAZARD POTENTIAL :

1. Man overboard.2. Finger injury / pinch.3. Slip, trip & fall.4. Fire or explode.

PREPARATION AND SAFETY EQUIPMENT :

1. Inspect all handling tools & equipment to be use, ensure all equipment is properly tools forthe job.

2. Ensure work area clean not slipper, clean your area before do a job.3. Use proper PPE & additional PPE [Work vest or safety harness] Inspect / check PPE condition.4. Conducted PJSM & review JSA / TRA before performing a job. Ensure all crews involved in a

task understood job description & safety matter.5. Open cold & hot work permit & ensure all crews already sign up the permit.6. Stand by fire watcher during welder performing a hot work.

OPERATION : Responsible

Procedure dismantle old anchor wire

1. Apply cold work permit, conducted PJSM with all crews involved & reviewSOP + JSA before the task begin.

2. Barge secured and tied up anchor onto the rack / handrail.3. Dismantle the anchor wire socket from the anchor.4. Install manila rope onto socket.4. Connect manila rope with BV crane air hoist & pulled the anchor wire

[ensure shackle installed in every corner / side ] till the end of wire position

at boat landing.5. Dismantle manila rope from end of wire.6. Stopper the anchor wire with chain. [ensure safety pin already install on

chain]7. Pull out the anchor wire from drum +/- 50 meters by the towing boat or

crane.8. Stopper the wire before cutting by torch.9. Lowering down the anchor wire onto deck barge & make a circle with

manual.10. Follow step no. 7 thru no. 9 until all anchor wire out from the anchor winch.11. Dismantle clamp end of wire & pull out the wire till drum winches empty.

Procedure install new anchor wire

1. Remove new anchor wire from store deck to 1st deck beside boat landing at

the near anchor changing position.2. Pull out new wire from spool connected to mooring line & pull the wire with

winch.3. Installed clamp of wire on the winches.4. Commence pull new wire anchor by winch till ends / finish.5. Install back anchor wire socket with anchor.6. Clean up work area after complete & closed PTW.
